📋 Instructions
  • ▸ One roster submission per team. Resubmit to make changes before the deadline.
  • ▸ Basketball: minimum 5 players required. Volleyball: minimum 6 players required. Maximum 15 for all sports.
  • ▸ Players may only appear on one registered roster per sport per season.
  • ▸ Jersey numbers must be unique within the team (basketball only).
  • ▸ Download the Emergency Contact Form, have each player fill in their emergency contact info and sign it, then upload the completed form before submitting.

Agreements & Waivers

⚖️ Waiver & Liability Release
By registering this roster with Hyper Action League Inc. ("HAL"), the team captain, on behalf of themselves and all listed players, acknowledges and agrees that participation in HAL activities involves inherent risks of physical injury, including but not limited to sprains, fractures, concussions, and other serious injuries. To the fullest extent permitted by law, each participant releases, waives, discharges, and covenants not to sue HAL, its officers, directors, employees, volunteers, sponsors, and facilities from any and all liability, claims, demands, actions, and causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by a participant or to any property belonging to a participant while participating in or preparing for any HAL event. This waiver is binding upon the participant and their heirs, executors, administrators, legal representatives, and assigns.
📸 Media & Model Release Agreement
The team captain, on behalf of themselves and all listed players, grants Hyper Action League Inc. and its authorized representatives permission to photograph, film, record, and otherwise capture the likeness, name, voice, and performance of all team members during HAL events and activities. This media may be used by HAL for promotional, marketing, educational, and commercial purposes across any media channels, including but not limited to social media, websites, print materials, and video content, without compensation or further notice. This release is irrevocable and applies worldwide for the duration of copyright protection. The captain confirms they are authorized to grant this consent on behalf of their team.
📋 Rules & Regulations
The team captain, on behalf of themselves and all listed players, acknowledges that they have read, understood, and agree to abide by all rules and regulations set forth by Hyper Action League Inc. This includes but is not limited to: league-specific gameplay rules for the registered sport, code of conduct and sportsmanship requirements, eligibility rules (one roster per player per sport per season), jersey number uniqueness requirements (basketball), roster submission deadlines, and any penalties or sanctions resulting from violations of these rules. HAL reserves the right to amend these rules at any time and will make reasonable efforts to communicate changes to registered captains. Continued participation constitutes acceptance of any amended rules.

Have each player fill in their emergency contact name, phone number, and signature. If the player is a minor, a parent/guardian signature is required.
